OCHIN’s Health Center Controlled Network

No-cost support that enhances clinical quality, patient experience, and care team well-being

As the largest of its kind in the nation, OCHIN’s Health Center Controlled Network (HCCN) leverages more than 25 years of knowledge solutions expertise to help participating health centers (PHCs) optimize their use of health IT to connect care teams and transform health outcomes for the communities they serve.

Our proven approach allows health centers to learn from OCHIN experts and each other, uncovering innovative solutions to common challenges. It includes dedicated health IT consulting tailored to support unique organizational needs and focused cohorts to foster peer-to-peer sharing, collaboration, and problem-solving.

How it works

HCCNs are collaborative groups of health centers addressing operational and clinical challenges related to health IT.

They undertake management, financial, administrative, and clinical initiatives that improve access to care, quality of care, patient safety, and cost efficiency. They also create economies of scale that support specialized training, technical assistance, data analytics, and group purchasing.

HCCNs receive federal funding from the Health Resources and Services Administration (HRSA). OCHIN’s HCCN received its first federal grant in 2012.

OCHIN’s HCCN guides health centers with strategies to improve health outcomes, drive efficiencies through automation, and innovate for the future of health care. We offer a variety of no-cost resources and dedicated consulting services that help PHCs get the most out of their EHR—regardless of platform—including:

Enhanced efficiency, continuous quality improvement

In July 2025, OCHIN received funding from HRSA to support federally qualified health centers (FQHCs) and FQHC look-alikes nationwide, reaching primary care sites that serve millions of patients in rural and metropolitan communities across the country.

Member eligibility requirements

Organizations may join any time during the current program cycle.
All FQHCs and FQHC look-alikes qualify.
There are no restrictions based on size or location.
Participation does not require use of OCHIN Epic EHR.
Enrollment takes only a few minutes.

Our PHCs can improve patient outcomes by taking part in peer-to-peer collaboration and health information exchange initiatives. These no-cost resources and activities help dedicated care teams use EHRs more efficiently, improve data analytics, and achieve a higher standard of patient care.

During the 2025-28 program cycle, Oregon Primary Care Association (OPCA) will partner with OCHIN as a sub-awardee. OPCA will provide additional support to Oregon PHCs by providing data aggregation support, as well as elevating priority issues shared by Oregon health centers.
