OCHIN Grand Rounds: A Health Systems Demonstration Project: The Impact of Patient Complexity on Healthcare Utilization and Outcomes”
April 12, 2019
12:00 - 1:00 pm PT
Presented by: Dr. Erika Cottrell, PhD, MPP, of OCHIN and Oregon Health & Science University
Research suggests social determinants of health may contribute as much or even more to health outcomes than clinical factors. However, care quality metrics typically emphasize disease-specific, clinical outcomes and do not often include social determinants. In the third phase of this Health Systems Demonstration project, OCHIN and Kaiser Permanente are collaborating to examine the impact of social determinants of health on healthcare utilization and outcomes. Most recently, the project is looking at how neighborhood social deprivation and individual chronic disease burden impact hemoglobin A1c control among diabetic patients and emergency department use among Medicaid-enrolled patients in Oregon. This Grand Rounds presentation will reflect on how OCHIN has collaborated with patient, provider, and health system stakeholders throughout all three phases of the Health Systems Demonstration project and we will share the latest findings.
