Four OCHIN member organizations were recognized last week at the annual OCHIN Learning Forum for their groundbreaking efforts to transform the health and well-being of patients, clinicians and communities.
At the heart of the Learning Forum—an annual enrichment event for members—was a celebration of member-led innovation. The 2025 Impact Awards honored four organizations that are redefining community health through the creative and effective use of OCHIN tools, technologies and insights.
During the awards ceremony, representatives from each honored organization shared success stories and reflected on the impact of their work. Their experiences highlighted the many ways OCHIN members are driving change—meeting local needs with solutions that improve care quality, foster patient health literacy and set new standards for care access.
The 2025 Impact Award recipients are:
Asian Health Services, Transforming Patient Health and Well-Being
Asian Health Services found a way to keep patients with hypertension connected to care—especially when visiting the clinic was a challenge. Through an innovative, physician-led telehealth and remote monitoring program, they provided iPhones and Bluetooth-enabled blood pressure cuffs to over 1,300 patients, breaking down barriers to consistent monitoring and setting a new standard of care using OCHIN solutions. The result: 78% patient re-engagement, improved health outcomes and significantly expanded access to care.

Lower Lights Christian Health Center, Transforming Patient Health and Well-Being
Lower Lights Christian Health Center enhanced patient care by addressing a longstanding challenge: access to specialty services. With OCHIN solutions, they improved referrals and strengthened MyChart adoption to reduce appointment wait times for patients with autoimmune disorders, who can now get on-site or telehealth appointments within 14 days—down from wait times as long as 10 months. They also introduced an on-site specialty pharmacy offering home delivery and partnered with other health centers to provide affordable medications.

Adapt Integrated Health Care, Transforming Clinician Health and Well-Being
Adapt Integrated Health Care knew supporting their clinicians meant delivering exceptional training. By analyzing their OCHIN Epic training program using data provided through OCHIN, they identified where providers were struggling. Rather than a one-size-fits all solution, they implemented personalized, at-the-elbow support that improved workflows, boosted efficiency, and reduced stress as well as time spent charting after hours. Now, clinicians are more confident, focused and better able to care for patients while also prioritizing their own well-being.

Lummi Tribal Health Center, Transforming the Health of Our Communities
Lummi Tribal Health Center is working to transform the health of their community members through an HPV immunization campaign launched in partnership with the Public Health Department. The Lummi Nation was the first tribe in the country to achieve 100% vaccination among eligible youth by using OCHIN Epic data to identify those not yet screened, while providers opened their schedules to support outreach. The initiative is raising awareness, providing critical education, and—with more clinics planned—laying the groundwork for a stronger, healthier future.
