In a new partnership with OCHIN, 11 of Iowa’s community health centers are collectively implementing OCHIN Epic to transform care delivery and better connect underserved communities with quality, whole-patient care.
Managed by INConcert Care, a sister organization of the Iowa Primary Care Association (Iowa PCA), the nearly state-wide collaboration serves as a national model for connecting community health centers in both rural and urban areas of the state with integrated support that allows for seamless coordination between medical, dental, behavioral health, and pharmacy services.
“We are honored to join forces with the Iowa PCA and INConcert Care to expand care access and positively impact lives through the collective knowledge and expertise of all three organizations,” said Abby Sears, CEO and president of OCHIN. “By working together to connect and strengthen the health care network in Iowa, we can ensure more equitable access to primary care for the long-term.”
Sears and Lindsey Haase, vice president of member and network success, joined representatives from Iowa PCA and INConcert Care in Des Moines, Iowa, last week. Together, the organizations reflected on the value of partnership in driving health equity and the important role that collaborative networks, which OCHIN has been connecting and strengthening for more than two decades, have in expanding access and maximizing patient outcomes.
