At Harbor Health, providing comprehensive, whole-person care is at the heart of the organization’s mission to help patients achieve their fullest health potential, especially those enrolled in the Program of All-Inclusive Care for the Elderly, or PACE. As the founder of one of the nation’s first community health centers, Harbor Health needed a modern, stable electronic health record platform capable of supporting the complex clinical, operational and regulatory requirements of caring for older adults.
When their legacy system could no longer meet evolving data and customization needs, creating the risk of system failures, Harbor Health partnered with OCHIN to develop and implement a unified platform designed to support PACE workflows, reporting requirements and care coordination.

In the video, Lorin Green, director of clinical applications at Harbor Health, shares how the organization worked with OCHIN to advance PACE-focused tools and services, helping providers deliver timely, coordinated and person-centered care while relieving administrative burden.
PACE programs help older adults who require nursing home-level care access a broad range of services while remaining in their homes and communities. These services, which include medical care, transportation, therapy and home-based support, enable participants to receive care in the settings where they feel most comfortable and connected.
Because PACE organizations operate within a highly regulated environment, this program demands robust documentation and reporting along with interdisciplinary collaboration. Since PACE combines elements of acute, outpatient and long-term care, it also requires specialized EHR functionality, greater interoperability, and tightly integrated clinical and administrative workflows.
Since transitioning to OCHIN Epic, both patient and provider satisfaction has improved at Harbor Health, and the care team takes pride in knowing they’ve helped elevate Epic’s PACE support to a new “gold standard.”
“The work we did with OCHIN brought our PACE tool to the next level,” Green said.
In addition to technology enhancements, Harbor Health also benefited from opportunities to learn from peers facing similar challenges. Through OCHIN’s collaborative member network, the organization gained access to shared insights, best practices and benchmarking data that informed ongoing improvements.
“It is nice to be able to reach out to other service areas,” Green said. “I can tap into those outside sources and say, ‘Hey, how did you do that? How did you make this more efficient? How are your numbers so great?’ You can share with a group of people at the collaborative.”
Harbor Health’s experience shows the value of combining innovative technology with a strong community of peers. By harnessing an interoperable and adaptable EHR platform and the collective knowledge of the OCHIN network, Harbor Health continues to strengthen care delivery for older adults, enabling clinicians to spend more time focused on care and helping patients stay connected to the services they need.
